Fees/Insurance
The fees refer to your out of pocket costs. Out of pocket costs include co-pays, co-insurance, deductibles and self-pay amounts for those not using insurance for services. What you pay is based on your insurance benefits unless you are self-pay. Payment is due at the time of Service. You will have the opportunity to pay prior to your scheduled service or at the start of session.
